Go to the nearest Municipal/City Social Welfare Development Office in your residence and submit your complete requirements for Persons with Disability ID application.

No. Successful online application does not guarantee ID issuance. The Persons with Disability ID issuance shall officially be accomplished in the nearest Municipal/City Social Welfare Development Office with your complete documentary requirements.
The PRPWD User Account gives the authorized Municipal/City Social Welfare Development Officers (MSWDO/CSWDO), Persons with Disabilities Affairs Office (PDAO), and National Council on Disability Affairs (NCDA) to issue Persons with Disability ID thru the PRPWD System.
Only the Municipal/City Social Welfare Development Officers (MSWDO/CSWDO), Persons with Disabilities Affairs Office (PDAO), and National Council on Disability Affairs (NCDA) who are authorized to issue Persons with Disability ID can request for PRPWD User Account. There is no need for the Persons with Disability who are requesting for ID issuance to request for a User Account.
2. At the Login page, click the Register and fill-out the Form.

Registration Successful! Please email an accomplished Service Request Form (SRF) to authenticate your registration at diseaseregistry.doh@gmail.com. Please click here to download the SRF.
7. The system will send/email you a message with your details.

3. Click Print button to print the Service Request Form.
4. Accomplish the Form and have it signed by your Head of Office.
5. Submit to us scanned copy of the signed Accomplishment Form at diseaseregistry.doh@gmail.com for verification of your request. (Note: The original document shall serve as your own copy.)
6. Kindly wait for an e-mail response from KMITS staff for 2-3 working days.
